Hospice Care At Home

At Santa Maria Hospice we believe that comfort, dignity and quality of life matter most during life’s final chapter. Our hospice care at home program brings compassionate physical, emotional and spiritual support directly to patients.

What is Hospice Care at Home?

In-home hospice care is designed to support individuals with a terminal illness in their chosen setting. For many, that means staying in the comfort of home. For others, it may be a family member’s residence, an assisted living facility or a nursing home.

Rather than seeking to cure, hospice care focuses on comfort, symptom management and emotional well-being, with an interdisciplinary team supporting both the patient and their loved ones.

Who is Eligible for Hospice Care?

A person may qualify for hospice care when they have a life-limiting illness with a life expectancy of six months or less, as certified by a physician. Common conditions include, but not limited to:

Indicators that are hospice-eligible can include but are not limited to:

  • Weight Loss
  • Increased assistance with daily activities
  • Increased weakness and fatigue
  • Frequent and multiple falls
  • Difficulty swallowing
  • Shortness of breath or chest pain
  • Continuous oxygen
  • Increase/changes in medication
  • Recurring or multiple infections
  • Frequent phone calls to physician
  • Frequent hospitalizations or ER visits
